There are a couple of “sound cue sheets (not in their original color, alas) for the mix of Apocalypse Now, drawn by Walter Murch; from the excellent new book by Michael Ondaatje, The Conversations: Walter Murch and the Art of Editing Film (New York 2002)” reproduced at the following link, along with a couple of interesting examples of song structure visualistions:

The Apocalypse Now sound cue sheets are an interesting historical document. These days we tend to edit and add notes directly into Pro Tools sessions using markers or clip groups as ways of logging our mix session rather than through separate documents.
